Within the Care in the Home division, we support adults with spinal cord injury, acquired brain injury and complex health needs to live in their own homes.

We'd like you to join us as a Support Worker at our clients home based in Crewe.

Meet the client:

Our male client has suffered a spinal cord injury, lives in his own bungalow and his family regularly visit him. He is easy-going and has a good sense of humour. He has his own vehicle that his support workers use to drive him to his university studies twice a week and to get out and about into the community.

When you'll be working:

  • 8am-8pm/ 8pm - 8am
  • 36 hours a week available

What you'll be doing:

This role is to support our male client who has a spinal cord injury. This will involve supporting with personal care, clinical skills of cough assist and oral suctioning, moving and handling, management of medication, driving the clients vehicle and domestic duties including supporting with cooking.

You will be required to drive our client's vehicle to take him between his home and university. Therefore you must hold a full UK drivers licence.

This role involves rapid mental health assessments, risk management, and collaborative care planning in partnership with A&E teams, crisis services, and the wider MDT. The postholder will ensure that patients receive safe, timely, and person-centred mental health support in line with Trust policies and clinical best practice. Key Responsibilities: Carry out comprehensive mental health assessments for patients referred via A&E or admitted to the inpatient unit. Triage and assess risk, including suicide and self-harm, and implement appropriate interventions. Work collaboratively with the multidisciplinary team to develop and implement care and discharge plans. Provide crisis intervention, de-escalation, and short-term therapeutic support where required. Act as a key point of contact between inpatient services, A&E, and community teams to ensure continuity of care. Maintain accurate and timely clinical documentation in line with professional and Trust standards. Provide clinical support and supervision to junior staff and students as appropriate. Essential Criteria: NMC registered Registered Mental Health Nurse (RMN) Minimum of 12 months' post-registration experience in acute or inpatient mental health services Strong skills in mental health triage, assessment, and risk management Understanding of mental health law and safeguarding procedures Excellent communication and clinical decision-making abilities Desirable: Experience in liaison psychiatry or A&E mental health triage PMVA or MAPA trained Familiarity with local care pathways and community resources
