Job Description and Person Specification - Support WorkerSupport worker Salary : £2,490 PMShift Pattern 2 days on 2 days off. Job Purpose: To provide relational support and life-skills training to young people living at our supported housing service, including the day-to-day running of the home in partnership with young people aged between 16 to 18. Acting as key worker to specific young people and taking responsibility for the household whilst on shift. The shift pattern consists of two days on and two days of and does incorporate sleep over at the properties during these times. Responsible to: Supported Housing Team Leader and Manager Responsible for: Young People residing at home. Key Outcomes: Young People are effectively supported. High quality support and relationships ensure that young people are kept safe, develop skills for life, positively challenged and supported to make informed decisions, leading to positive outcomes. Young People have access to high quality support. This is achieved by developing and sharing best practice. Working with the Manager to ensure the accommodation meets the required standard and contributes to positive self-esteem and expresses the value we place in young people. Young people are provided with a range of educational, social, cultural and fun opportunities to develop positive self-esteem, resilience and confidence. Main duties and responsibilities Working with young people Build quality relationships with young people. Ensure that all young people receive a high-quality service. Help with housing management and appropriate general support. Assist in assessing and interviewing prospective young people for the service. Work with young people with an agreed personal support plan. Provides opportunities for young people to take part in life skills programs. Ensure young people have opportunities to be involved in the running of the house through house meetings, and by responding to young peoples' issues and concerns. Assist young people in identifying suitable move-on accommodation. Enable young people to claim appropriate benefits. Produce and oversee all paperwork for the young people you key-work, undertaking regular key work and personal action planning. Reacts to, and reports incidents, and issues first warnings to young people as appropriate To provide appropriate assistance to young people in emergency situations (including outside normal working hours), and to arrange/co-ordinate the involvement of other agencies i.e. police, social services. Carry out full admission, induction and exit interviews. To assist with young people's meetings and take notes. Work within the standards and guidelines of any regulatory body relevant to the area of work e.g. OFSTED. Input data, including session notes, support plans and incidents, into an online recording keeping system. Housing Management and Maintenance Take inventories of furniture and equipment when residents move in and out. Keep properties maintained, furnished, and managed to meet health & safety standards and insurance requirements. Assist resident in maintaining cleanliness in their flat/room. Assist residents to maintain the safety and cleanliness of the properties. Maintain and check the security of the property. Check furnishings and equipment, and report repairs to ensure that it meets health and safety standards. Clean and keep all communal areas tidy to a specified standard. Report items requiring repair or maintenance. Carry out minor repairs, and maintenance tasks including changing light bulbs in line with procedures. Health and Safety Work with the highest regard to safety, and to minimise risk, taking responsibility for the welfare and safety of residents. Assist in working with other agencies to assess and manage risk relating to prospective and current residents. Ensure that risk assessments are properly carried out, and appropriate action taken. Ensure that risk assessments are in place and regularly reviewed and updated for the young people you key-work. Ensure that fire safety equipment is working and carries out fire drills. Receive and hand over information at the beginning and end of each shift, where appropriate. 2. Delivering services Monitor and evaluate work to ensure services are delivered to action plans, quality and financial standards. Build links with key individuals in other agencies to ensure good communication about individual residents. To maintain a good relationship with neighbours and the surrounding community. Liaise with other agencies, advocating the needs of young people where necessary using a person-center approach. Ensure the development of best practice within accommodation. 3. Responsibility for the safety of staff, volunteers and public in contact with Upwards Care Ensure that safeguarding of children and vulnerable young people is carried out and promoted through every piece of work Upwards Care does. Ensure that Upwards Care policy and practice is implemented to a standard that meets health and safety regulations with the highest regard to the safety of young people, employees, volunteers, students and hosts. Maintain Upwards Care and Partner Agencies premises and equipment to a high standard. 4. Financial responsibilities Manage work within agreed budgets to ensure that Upwards Care services meet financial targets. Assist the team to implement the marketing and fundraising plan for Upwards Care projects. Work within the financial rules and regulations as approved by the board and set out in the Financial Regulations Policy. 5. General Abide by Upwards Care's code of practice, equal opportunities and other relevant policies. Embed a culture and practice of reflection and evaluation across all work. Contribute to service development by making suggestions and expressing views to Line Manager. Ensure the health & safety of all work is well managed, following policy and procedures and appropriate risk assessments are carried out. Ensure that tasks are carried out effectively and all records are kept up to date. Meet targets and deadlines according to agreed Key Performance Indicators. Participate and contribute to supervision sessions with Line Manager. Attend and contribute to team meetings and other events when required. Participate in professional development training as agreed with the Line Manager. Develop positive working relationships with other Upwards Care workforce. Represent Upwards Care in a professional and positive manner. 6. Working environment The post holder will be expected to work 40 hours per week rotationally, mainly in the home. There may be opportunities to work on other days in other homes in the allocations. As part of this role the post holder will be expected to work to cover training, supervision, and meetings and anything else deemed appropriate by your Line Manager. There is an expectation that when needed workers may be asked to work in other Upwards Care Housing within the area. The post holder will be expected to travel within the locality and occasionally elsewhere. The post includes lone work with on-call support from management. A degree of flexibility is needed, and the post holder may be required to perform work not specifically referred to above. Such duties however will fall within the scope of the post. Full DBS check will be required, company will pay for this. 7. Person Specification: Your application will be assessed throughout the process against the following criteria. Research findings and statutory guidance and provide and present concise reports about complex information in line with evidence-based social work practice to meet the requirements of the Local Safeguarding Board, the Court, and other regulations. Manage a complex and high-risk workload and build professional relationships with children and their families to safeguard children and promote positive outcomes. Communicate effectively with children and their families to ensure their views and wishes are heard and provide written reports about their circumstances and plans for intervention. Manage the Social Work pod, prioritise and allocate tasks, provide guidance, direction, motivation, and support to ensure effective services to children and their families. Provide professional supervision to social workers and children s practitioners to ensure accountability for social work practice and assist with the professional development of staff within designated practice standards. Liaise and work with other agencies and promote joint working in line with statutory guidelines to ensure an integrated approach and multi-disciplinary working for the benefit of children and their families. Manage budgets for the commissioning of services and authorise cash payments to families in line with Financial Regulations to ensure value for money and appropriate assistance to families. Manage the child s record using Cheshire East's recording system, provide aggregated information and statistics, and share information with other agencies in line with the requirements of the Data Protection Act to promote the welfare of children. Contribute to the development of social work policy and procedures to ensure that services best meet the needs of children and families. Ability to analyse performance data, motivate and mobilise staff to ensure that performance targets are met and maintained consistently across the service.
